FMCA On-Line Submission Form for Music Club Annual Information

The Music Club Annual Information is now submitted to the FMCA using an on-line form (Google Forms).
The on-line form can be completed on many devices such as mobile phones, tablets (Android/Apple) or desktops (PC/Apple).

  1. What if I don’t want to use my email address for security reasons?
    The person completing the form is required to have an email address, so the completed form can be emailed back to them for checking. You can use another person’s email (with their permission) or you can use "web@fmca.org.au" and I can send a copy of your form to you for checking.
  2. What if I don't know the answer to a question and a response is required?
    Just enter "not known".
  3. What can I do if I didn't receive the completed form response email from Google?
    Check your "SPAM" folder for an email from "Google Forms".
  4. How safe is Google Forms?
    The data is as secure as most other systems which take survey data and store it. Yes, Google Forms responses are stored in a worksheet that can only be accessed through the FMCA Google account login.
  5. Where is Google Forms data stored?
    All responses to questions are saved in a spreadsheet, and stored in the FMCA Google Drive.
  6. What is the difference between Google Forms and Google Docs?
    Google Forms is a survey tool that's part of G Suite - Google's complete office suite (although some people refer to it all as Google Docs). The other main services included in the cloud-based suite are Sheets (Excel), Docs (Word), and Slides (PowerPoint).
